In a Coupon field on a Gravity Forms checkout covered by the coupon’s settings. This recipe uses the official Gravity Forms Coupons Add-On.
Set the coupon’s usage limit to one in the snippet configuration. That limits redemption of that code; it does not prevent someone requesting another code through the signup form.
One free snippet, configured once with the form, Unique ID field, Gravity Forms coupon destination and discount settings. The code’s format and notification emails are ordinary form settings.
GP Limit Submissions can limit completed submissions by logged-in user, email value or IP. An email can be changed and an IP can be shared, so choose a rule suited to your audience; none of these alone proves a unique person. Coupon redemption limits are a separate setting.
Add GP QR Code and use the Unique ID merge tag’s :qr modifier in the confirmation or notification. Enable QR Code Scanner on the checkout’s Coupon field so scanning fills in the code. The coupon’s configured usage and eligibility rules still control whether the discount applies.
