Does leaving out names make the survey anonymous?

It avoids collecting direct identifiers such as a name or email, and the location stops at the city. Disable IP storage and review other entry metadata, server logs, analytics and exports before describing the results as anonymous. Rare combinations of salary, location and demographics may still identify someone; review and aggregate results before publishing them.

Does the location go to Google?

Address suggestions send location requests to Google’s Places API using your key. Display the required Google terms and privacy links. Separately review any spreadsheet or analytics exports you enable; GC Google Sheets sends the fields you map to the connected sheet.

Why sections instead of pages?

Thirty questions with two branches in one form was the maze this build replaced. As seven sections, respondents open the ones that apply to them in whichever order they like; the community’s own verdict was “choose your own adventure”.
